[daisy] PDF renderer unification

Hello list,

We changed the book rendering engine from IBEX to FOP.
Everybody using the books feature should read the following wiki
article:

The article links to two versions of the daisy documentation,
there are some differences.  If any of these problems cause major
problems for you, or if you spot other differences that do, please place
your remarks in this thread.

(These changes have been spotted using evince and xpdf)

* Front page font
IBEX = serif
FOP = sans
* Bookmarks
IBEX: pdf opens with bookmarks collapsed
FOP: pdf opens with bookmarks expanded
* TOC
IBEX: uses ... dots between titles and page numbers
FOP: uses ___ lines
* TOC links
Only work for FOP (using xpdf)
* footer
The separating line seems heavier with FOP
* page 26, link to documentation of other releases
IBEX: shows 'page 0',
FOP: shows 'page      '
* page 26, link 'published as a daisy book'
Works in neither version, but behaves slightly different in both
versions (xpdf)
* page 27, link to 'source code (page 41)'
for xpdf, only works in FOP version
* page 359: table lines
IBEX: some lines look heavier (may be caused by specific PDF viewer?)
FOP: all lines same weight
