[daisy] PDF Generation and Table Images

There are no errors in the cocoon log and the images appear when outside 
of the table. It appears that if I upload the image from my hard drive 
and it is stored in the repository then it does not get generated in the 
PDF document. But if I link to an external image, then that image will 
get generated in the PDF.
