[daisy] guestbook on daisy 2.0.1

Hi,

I try to get the guestbook extension to work.  I get next error :
For http://localhost:8888/daisy/testsite2/ext/forms/guestbook , i get 
next error:
There is no site called "daisy".

For http://localhost:8888/testsite2/ext/forms/guestbook , i get next error:
Error Resource not found.
file:/C:/app/daisy-2.0.1/daisywiki/webapp/daisy/sites/cocoon/forms/sitemap.xmap 
doesn't exist.
C:\app\daisy-2.0.1\daisywiki\webapp\daisy\sites\cocoon\forms\sitemap.xmap 
(Het systeem kan het opgegeven pad niet vinden)

Can i get some help?
