[daisy] Adding a customized document to Select Document Type

Hi,

I am trying to add a new document type to the "Select Document Type"
screen.  The problem is the document type is custom made, based off the
example "Creating a new document with custom initialization" found at
http://cocoondev.org/daisydocs-2_0/374-cd/60-cd/297-cd.html. 

In other words, I have a script that pulls a document together and also
adds some content (which I'm using as a template).  Right now the only
way to reach this "new" document type is to go directly to the URL.
This is fine if I want to link to it in the navigation or in a doc
somewhere, but I want it to appear in the "Select Document Type" screen.
Any clue where that screen is generated from or other thoughts?
