[daisy] Changes to read access details (partial read access)

Hi,
Based on feedback from colleagues, I've made two changes to the read
access detail permissions:
- there is a new detail permission that allows to control access to the
fulltext context fragments separately from the fulltext index. So you
can now allow users to search the fulltext index, without being able to
see the context fragments.
- the evaluation behavior for the access details has changed a little
bit to make it more obvious: if you grant read access without
configuring specific details, this means you grant full read access.
Before this meant that the access details were in the state 'leave',
causing earlier access details to be reused. Now you have to explicitly
put them in state 'leave'.
Especially the second change might affect current trunk users who are
making use of the partial read details.
