Currently the menu (containing the various actions / non-document navigation) is simply constructed in the XSL (menu.xsl + page specific XSLs for the page-specific menu).

Instead, we could have some menu-description file so that:
 - menu items pointing to extension functionality can be added (without modifying / independent of the skin)
 - menu items for functionality to which the user does not have access could be removed (see also DSY-480)
