If the tool is not fixed (I can't provide a patch, too) and since this only affects <2.0 installations that make use of cross-references (i.e. use daisy books), a notice in the migration documentation would be helpful. To fix all cross-references manually, do a full-text search for 'ref*daisy*' and add the server namespace where it is missing.
