[daisy] JS errors when trying to add collections to a document

When trying to add a collection to a document, under the 'Misc' tab, I
get the following error in the error console of FF:
Error: from.options has no properties

Clicking this error displays the JS file with the cursor on the offending line:
> for (var i=0; i<from.options.length; i++) {
The selected collection is not added to the right side and none of the
tabs along the top are clickable after this (Parts, Fields, etc).
We have just finished doing a 1.41 to 1.51 upgrade (big job redoing
all ours skins) and everything appears fine except for this problem.
We have made changes to layout.xsl for our custom skin, as well as
document-to-html.xsl and navigation.xsl but I can't see how any of our
changes could be causing this error (yes famous last words).
On an unrelated matter, is it possible to put htmlcleaner.xml into the
daisywikidata directory or do I have to edit the the "webapp" version?
